Permission Release

I, parent or guardian of the below named child, give my approval for my child to participate in any and all activities at Tip-Top Brain. I further hereby release, indemnify, and hold harmless the organizers, instructors, and staff of Tip-Top Brain. I likewise waive, to the extent not covered by liability insurance, any claim against any person overseeing my child’s activities and transporting my child to and from activities.

Credit Card Details + Refund/Make-Up
Please input credit card details. By entering your credit card information below, you authorize Tip-Top Brain to process your child's summer camp tuition to this credit or debit card. You will be charged the amount agreed upon (program of choice) with the program coordinator. A receipt for each payment will be emailed to you and the charge will appear on your bank statement. You agree that no prior-notification will be provided unless the date or amount changes, in which case you will receive notice from us at least 10 days prior to the payment being collected. All payments are final, no refunds will be issued. No make-up days will be granted under any circumstances.
